Team Roster Submittal Form

     Please complete the following form in its entirety before clicking the "Submit" button.  If you have fewer than 8 players this month, enter "None" in the Name field for the unfilled positions.  Please enter an index only for players who DO NOT have an MSIS index.  You may check to see if a player has a MSIS index by clicking here.

    The form will not allow a submittal until correct data is entered in each field.

    If you want a return receipt and confirmation that your roster went through, be sure to complete the 'Submitter email' field.
